For patients battling severe autoimmune diseases—specifically anti-Jo1+ antisynthetase syndrome (ASyS) and diffuse cutaneous systemic sclerosis (dcSSc)—conventional treatments often fall short. A recent approach, detailed in a compassionate use programme at the University Hospital of Düsseldorf, explores the potential of targeted therapies, blinatumomab and teclistamab, alongside maintenance rituximab (RTX), for these critically ill individuals.

A Last Resort for Refractory Illness

Understanding the Patient Cohort

The treatment programme involved ten patients—five with ASyS and five with dcSSc—all of whom had exhausted standard treatment options. Every patient with ASyS had previously failed to respond to at least four immunomodulatory medications, including RTX. Similarly, all patients with dcSSc had not responded to at least three different immunomodulatory/antifibrotic drugs. The Challenges of ASyS

These included muscle weakness, mechanic’s hands, dyspnea, and, in some cases, interstitial lung disease (ILD). One 65-year-old woman, diagnosed in 2022, had a prior hepatitis B infection and experienced flares requiring multiple courses of prednisolone despite prior treatment with methotrexate, azathioprine, high-dose IVIG, and RTX. Another patient, a 36-year-old man, developed both ILD and myocardial involvement after RTX treatment, requiring multiple interventions including cyclophosphamide, MMF, tacrolimus, and tofacitinib.

The Complexities of dcSSc

The five patients with dcSSc all exhibited widespread skin fibrosis, ILD, and heart involvement. A 65-year-old man, diagnosed in 2006, had progressive ILD despite treatment with cyclophosphamide, azathioprine, and MMF. Another patient, a 46-year-old man, experienced a rapid decline in lung function and new onset of biventricular systolic dysfunction. A common thread among these patients was significant weight loss, with some losing up to 18 kg since their diagnosis.

A Novel Treatment Approach

Blinatumomab, administered via continuous intravenous infusion, and teclistamab, given subcutaneously, were employed with specific premedication protocols to manage potential side effects.

Maintaining Remission with Rituximab

To prevent the re-emergence of autoantibody-producing B cells, rituximab (RTX) was administered following the initial course of blinatumomab or teclistamab. The RTX dosing schedule was individualized, considering the patient’s response and the pharmacokinetics of the prior treatment. In patients with SSc, RTX was initiated two weeks after the final teclistamab dose, demonstrating complete suppression of peripheral B cell counts and a decline in autoantibodies.
