Sequential Radiotherapy and Immunotherapy May Improve Survival in Advanced NSCLC

Sequential Radiotherapy and Immunotherapy May Improve Survival in Advanced NSCLC

June 17, 2026 discoverhiddenusacom Health

Sequential immunotherapy and radiotherapy are linked to longer survival for patients with newly diagnosed advanced non-small cell lung cancer (NSCLC), according to a study published in JAMA Oncology. The OCEANUS study reported median overall survival of 20.3 months for sequential treatment, compared to 16.0 months for those receiving concurrent treatment.

Why does the timing of radiotherapy and immunotherapy matter?

Treatment sequencing may influence patient outcomes in advanced NSCLC. For patients with newly diagnosed advanced disease, the study found that sequential immunotherapy and radiotherapy were associated with a 32% lower risk of death compared to concurrent delivery. This result was statistically significant with a P value of .045 and an adjusted hazard ratio of 0.68.

The investigators also found that chemotherapy was associated with longer survival in patients with newly diagnosed advanced NSCLC. These findings suggest that systemic treatment partners remain a relevant part of the strategy even when combining radiotherapy and immunotherapy.

Did You Know? The OCEANUS study analyzed real-world data from the Hong Kong Hospital Authority Clinical Data Analysis and Reporting System, which covers more than 90% of the population.

How did the OCEANUS study evaluate NSCLC treatments?

Researchers analyzed data from patients diagnosed between January 1, 2010, and December 31, 2021. Of the 3,522 patients who received immune checkpoint inhibitors (ICIs), 335 also received radiotherapy and were included in the main analysis. This group consisted of 155 patients with newly diagnosed advanced NSCLC and 180 with refractory NSCLC.

The patient group had a median age of 64 years, and 73.7% were men. To reduce selection bias, the investigators used propensity score–based methods, specifically overlap weighting and inverse probability of treatment weighting.

Expert Insight: Samantha Carter notes that using real-world data is critical because these patients often have variable disease burdens, comorbidities, and performance status differences that aren’t always captured in the highly selected populations of randomized clinical trials.

What happens next for lung cancer treatment sequencing?

The study’s findings in refractory NSCLC were less definitive. Radiotherapy followed by ICI maintenance showed a numerical survival advantage, with median survival of 11.2 months compared to 6.7 months without maintenance. However, this difference was not statistically significant (P value .20).

Because this was an observational cohort study, the results are considered hypothesis generating. Future steps may include prospective randomized trials to confirm the optimal sequencing of these therapies and to clarify the specific role of chemotherapy and ICI maintenance in different clinical settings.

Frequently Asked Questions

What was the survival difference for newly diagnosed advanced NSCLC?
Median overall survival was 20.3 months for the sequential group and 16.0 months for the concurrent group.

Did chemotherapy improve survival in refractory NSCLC?
No, the addition of chemotherapy was not significantly associated with improved real-world overall survival in patients with refractory disease.

Is the survival benefit for refractory NSCLC confirmed?
The survival benefit for ICI maintenance after radiotherapy in refractory NSCLC was numerical but not statistically significant, meaning the result should be interpreted cautiously.

Do you believe real-world data should carry as much weight as randomized clinical trials in shaping treatment guidelines?
